The 21-year-old student came to the clinic because of acid heartburn and vomiting. Her medical history also included an irregular menstrual cycle.
Associated symptoms:
Headaches that are relieved by vomiting.
The symptoms worsen with irregular meal times. If she sticks to her regular meals she is fine, but her university timetable does not allow regular mealtimes. Three to four days before her period she gets abdominal bloating and feels very unwell. Stress makes the symptoms worse, for example before exams. Not eating helps.
What does 'unwell' mean?
“I cannot bear tight clothing; I have to loosen my clothes.”
“My whole lower abdomen is bloated and it goes up to here. I feel so uncomfortable until I have released the air. I then have to belch loudly.”
She is nervous about her exams and worries whether she will do well enough.
“I am very good at my studies, but I am afraid I might fail.”
“I am known for my good results, but what if I get bad grades?”
“When I am in a lecture and another student gives the right answer, I immediately feel inferior.”
“I am always tense, constantly thinking about things.”
“What will people think of me?”
“Sometimes I have feelings of inferiority and am unhappy with my appearance.”
“I care that people appreciate me for my good performance.”
“If I have made a mistake I can admit it, but if not, I do not let up until the person opposite me breaks down in tears.”
“I feel weak inside.”
“My problems have worsened since I no longer get on so well with one of my cousins. He always meant a lot to me and we were very close. But then he got married and his wife did not want to tolerate our friendship. That's why we no longer see each other.”
“I would have liked to have had a big brother; he could have protected and spoiled me.”
“If I cannot maintain my level of performance, I feel guilty. I cannot look people in the eye anymore.”
“If I fail at university, my image suffers.”
“I like to stand out from others and I am pleased when I manage to be different from others.”
“For a blood transfusion I was chosen as the only one out of 50 girls. That's quite an achievement.”
“I like being successful, but at the same time I am afraid of losing control.”
“I have severe flatulence. My whole lower abdomen is bloated and I only feel comfortable again when I can release the gas in my belly.”
“I am not like other people, I cannot enjoy life to the full. I feel confined and have the sensation of suffocating. It is as if there is not enough space.”
“I have the feeling of being in an uncanny place, without light and without air; pressure comes from both sides and there is no way out.”
Cannot enjoy the following things:
“All my friends go out and have fun. I don't want to join them because they usually go out to eat and my problems always start after eating.”
“I do not feel well because I – unlike my friends – cannot do anything that I enjoy.”
“I would like to lead a life in which I can do everything I enjoy without problems. But after a while I get fed up with it again and I wonder what people will think of me… she is having so much fun.”
“Sometimes I think that success might make me very selfish.”
Say a few more words about this uncanny place.
“It reminds me of the Kalapani prison or of a womb.”
“I have to get out of there as quickly as possible or I will suffocate.”
“Even now, when I talk about it, I have this feeling of suffocating.”
Womb?
“Like a baby in the mother's womb, it is also dark there and the child can only communicate through the mother.”
How does it feel in the womb?
“Very good, very protected.”
“For a while that is OK, but then it becomes too tight.”
What happens when it becomes too tight?
“Suffocation, it will try to get out as quickly as possible.”
Describe this feeling of suffocation.
“There is intense bloating and I feel very unwell, as if I am suffocating.”
“I have to let the air out explosively and then I feel better.”
