This case report concerns a 45-year-old man. For six years he has suffered from facial paralysis; the trigger was a left-sided otitis media caused by cold wind. He can only close his left eye with difficulty and can scarcely lift the left corner of his mouth. He is also photophobic. His sense of taste has changed and is impaired. Six months earlier he had severe pain in the left side of his face. Other complaints are: allergy to dogs, cats and grasses, with sneezing and itching eyes. He had already been operated on because of his blocked nose. He has digestive problems with very hard stools. Premature ejaculation. Every day he has headaches in the temples, which are made worse by cheese, fat and eggs. His hands are weak; he notices it when he exerts himself physically. The patient history records recurrent otitis with tonsillitis that occur when the weather turns cold.
Life situation: The patient was born in India and came to Holland in 1983 to gain professional experience in his uncle’s restaurant. He is the youngest of six siblings. His father died when he was three years old. His mother died in India shortly before he was to see her again after nine years abroad. He formerly worked for a wholesaler of women’s outerwear, but left the company after an argument with his boss. He suppresses his anger about that. Afterwards he opened a small snack bar, which he had to close again because of vandalism in the neighbourhood. He currently works as a driver for a bakery and is glad that he can be home with the children in the afternoons. His great interest remains the textile industry. His marriage broke down three years ago; he has two children, aged seven and ten. He and his wife often argued, mostly about sex. In addition, his wife’s family constantly interfered. His wife was often angry and he eventually left her. For two years he broke off contact with everyone. He says he has a poor memory and cannot remember his past well. The mess made by his children often annoys him and makes him angry, but he keeps his anger to himself.
Impression: An Indian man, small in stature with rather flaccid tone. Soft, gentle voice, pitch somewhat high. He gives a rather weak impression – type 'underdog'.
