Out of breath and afraid of falling after an accident

News

A. is a 70-year-old, pleasant old man, slightly overweight and, despite his age, cheerful and lively. His face is puffy and bluish. He suffers from chronic bronchitis, experiences sensations of suffocation and notices the slightest lack of oxygen in a room. In the mornings he feels as if he had drunk 2 litres of wine. He becomes short of breath easily, worse on exertion such as climbing. His legs feel heavy. He feels better in the afternoons.
All his problems began 8 months ago after he fell 2.5 m from a ladder. At first he took it lightly and merely checked whether he had broken anything; he sat on a bench in his garden with a beer in front of him. Since then, however, he lives in constant fear of falling. At home he clings to the handrail at the top of the stairs and can only go down the stairs with the help of the handrail. He has stopped cycling because he fears a fall.

This is rather uncharacteristic for him because he was always a daredevil who never feared such things. He was a successful businessman heading a large family company, which he sold when he got older. He now enjoys his retirement. He likes to drink beer, up to 4 bottles an evening. He snores very heavily, so his wife prefers to sleep in another room. His blood pressure is elevated.

He could not tolerate antihypertensive treatment because the medications made him dizzy. He cannot stand doctors and generally looks after his own health. He drinks mistletoe tea to keep his blood pressure normal. I advise him to reduce his alcohol consumption because I suspect he has sleep apnoea.

Analysis:
The whole story of "I haven't felt well since the fall" with the subsequent anxieties, his bluish appearance and his snoring first makes me think of Opium, with a second thought towards the carbons and the Asteraceae. To confirm this, I question him about his pain: "I am completely insensitive to pain. I think my abdomen could be opened without anaesthetic; but I cannot wear varifocal glasses, they give me headaches."

He is colour-blind, but selects blue 15D and yellow 1C without difficulty.

That suffices as confirmation for Opium (they favour 2C, which is very close to 1C).

Prescription: Opium 1M, 2 doses within 2 days

From the first night he slept better and snored much less. His wife says that he looks completely different and much better. After a few days the swollen, bluish face as well as the drunken feeling in the mornings had disappeared. He wishes to continue taking the remedy and now takes it twice weekly.

After 3 months his improvement had progressed further and he returned to his former self. His legs no longer feel so heavy, the snoring and the bronchitis have ceased, and he can go down the stairs without the handrail. After 6 months he is still taking Opium 1M and feels well. He has reduced his alcohol intake. He is pleased that his case is being published!
